Billing-unit calculator

Convert an administered dose to HCPCS billing units and see the arithmetic: rounding, vial utilization, and the quantity left over from single-dose containers.

G0480 — Drug test(s), definitive, utilizing (1) drug identification methods able to identify individual drugs and distinguish between structural isomers (but not necessarily stereoisomers), including, but not limited to gc/ms (any type, single or tandem) and lc/ms (any type, single or tandem and excluding immunoassays (e.g., ia, eia, elisa, emit, fpia) and enzymatic methods (e.g., alcohol dehydrogenase)), (2) stable isotope or other universally recognized internal standards in all samples (e.g., to control for matrix effects, interferences and variations in signal strength), and (3) method or drug-specific calibration and matrix-matched quality control material (e.g., to control for instrument variations and mass spectral drift); qualitative or quantitative, all sources, includes specimen validity testing, per day; 1-7 drug class(es), including metabolite(s) if performed

From the HCPCS descriptor, e.g. “10 mg” for “Injection, rituximab, 10 mg”.

2 · Dose administered

The amount given to the patient for this line.

3 · Container (optional)

Blank uses the minimum needed for the dose.

Optional. Overrides the container-based estimate.

Container type

4 · Payment limit (optional)

Leave blank to skip the derived dollar estimate.

Verify before billing

Payer rules and claim requirements must be verified. This calculator applies arithmetic conventions described in Medicare instructions — whole units, rounded up when a dose is not a multiple of the descriptor — and other payers may differ.

About discarded-drug modifiers (educational): for separately payable Part B drugs from single-dose containers, Medicare instructions describe reporting discarded amounts on a separate line with the JW modifier and using the JZ modifier when nothing was discarded. CodeRx does not determine whether a modifier applies to your claim. Check the current CMS discarded drugs guidance, your MAC, and the payer.

Vial optimizer

The combination of container sizes that leaves the least drug unused, and whole-container doses within a rounding threshold. Sizes come from the CMS NDC crosswalk for this code (billable units per item × billing unit).

Container sizes stocked

No crosswalked package sizes for this code — add sizes below.

Use your institution’s policy. The HOPA 2017 position statement recommends rounding monoclonal antibodies and other biologics to the nearest vial within 10%.
